How to Submit Forms
Unless otherwise stated, you can submit completed forms and other required documents to the Secure Document Portal by clicking the button below.
If you choose to submit forms to reguwt@uw.edu, ensure that you are sending from your UW email address, unless you no longer have one (e.g. former students, newly admitted students).
If you have a "No" on your Directory of Release and you email us from a non-UW email address, you may be asked to send us a copy of your photo ID for verification of your identity.

DegreeVerify - Online College Degree Verification Services - Third party entities or students not attending may purchase enrollment information or verify degrees through the National Student Clearinghouse.
Student Self-Service Enrollment Verification - Free access for current students - Includes dates of attendance, name of major and CIP code of major
