  • Use this form for groups of 10 or more from the same company.  The discount rate will apply.

Cost

• $375 per person
• $325 per person (10 or more people from the same company).  Discounted registrations must be received at the same time.
• $395 per person (after April 25, as space is available)

Refund Policy

If you register and cannot attend, a 100% refund of the registration fee less a $35 processing fee will be granted if requested at least two weeks prior to the event, April 25, 2012. If you cancel within two weeks of the event, you will receive a 50% refund of your fee, less the $35 processing fee. No refunds will be granted to attendees who fail to cancel prior to the day of the event. If you are unable to attend, you may send another person in your place.

Terms and Conditions

The Milgard School of Business reserves the right to cancel the event for any circumstances beyond its control. The total amount of liability of the University will be limited to a refund of the registration fee. Attendees who incur any expenses (including but not limited to nonrefundable airline tickets) in anticipation of attending the event do so at their own risk. Submission of this registration form acknowledges acceptance of these provisions.
