Applicant forms

The BSN application requires a number of materials, which are detailed in the BSN Program Application and Supplemental Materials Kit (BSN Kit), which will be available in October.

In addition to the following forms, applicants must submit a current résumé and a very specific personal essay!   See the BSN Kit for complete instructions.  Please note that the BSN personal essay differs from the Undergraduate Personal Statement, which is part of the online transfer application to UW Tacoma.  Again, see the BSN Kit for details. 

BSN applicants must also submit a UW Tacoma Transfer Student Application and an official transcript for every college/university attended (even if no credits were earned).  See the Admissions website for more information.
