Health Sciences immunization requirement for Social Work/Welfare students

Q: Why do I need all of these vaccinations?

A: These new standards are based on the Center for Disease Control (CDC) requirements for health care providers and are more stringent than the existing UW Tacoma measles vaccination requirements.

Q: Why can’t the University just use the immunization information that was requested at the time of admission?

A: Again, the new standards are based on the CDC requirements for healthcare providers and are more stringent than the existing UW Tacoma measles vaccination requirements. The Health Sciences immunization policy is totally separate from the University of Washington’s measles immunization policy for all UW students, regardless of their area of study.Files

Q: Can I get a copy of my documentation for MMR that I supplied to the Registrar at the time of admission?

A: No. The registrar is not providing copies of this documentation to students. For more information contact 253-692-4400 or visit Mattress Factory 253.

Q: Do I need to provide proof of health insurance as part of this process?

A: If you have health insurance you may be asked to provide proof, however it is not a requirement that you have health insurance. It is highly recommended that students look into obtaining coverage from the UW Student Health Insurance program or through another carrier, however it is not required.

Q: Will I be charged an administrative fee from HSIP for the verification process?

A: Yes, an administrative fee will be charged to each student annually by the Health Sciences Immunization Program (HSIP) to ensure proper tracking and compliance policies are followed.

Q: What paperwork is required?

A: All MSW and BASW students must complete and submit a 2013–2014 required immunizations form along with all of the required documentation to myshots@uw.edu.

Q: Where do I find the 2013–2014 required immunizations form and additional information?

A: Please refer to the Health Sciences Immunization Program website for additional information.

Q: What is the submission deadline for current BASW and MSW students?

A: All MSW students entering field placements during summer quarter 2013 or autumn quarter 2013 must have their documentation in and verified no later than June 21, 2013 or they will not be permitted to begin their field placement. All BASW students entering field placements during autumn quarter 2013 must have their documentation in and verified no later than September 24, 2013 or they will not be permitted to begin their field placement.

Q: I am a new student to one of the Social Work programs (MSW or BASW). When do I need to submit immunization paperwork to myshots@uw.edu?

A: You will be advised of specific dates upon admission, but generally all documentation must be supplied prior to entering the program.

Q: Can I drop off my paperwork with the Social Work Program?

A: No, unless your practicum coordinator (Tom Diehm or Teresa Holt) specifically asks you for a duplicate copy, no forms should be dropped off in the Social Work Program office. All forms and documentation must be submitted electronically to myshots@uw.edu. If you are unable to send your documents electronically, please contact the HSIP for an alternate method at myshots@uw.edu.

Q: Does UW Tacoma offer immunizations on campus?

A: Yes. Students should visit Student Health Services located at 17th and Market streets for a list of available vaccines, titers and pricing information. They can be reached at uwtshs@uw.edu or 253-692-5811.
