SUS COVID Reporting Protocol:
1. Using the “When do you report close contact exposure or Covid- 19 illness” decision tree
2. If, via the decision tree, you need to report a positive case:
- Email: Students report positive case to Dr. Bernard Anderson, bander48@uw.edu and personnel (faculty & staff) EHS, covidehc@uw.edu
3. If you need to change how your course meets (e.g., you are waiting for test results; there is a positive case in class)
- Notify your students about that change (e.g., synchronous online)
- Email Lisa as notification of change (hoffmanl@uw.edu)
4. If there is a confirmed positive case from your class that EHS contact tracing reports to you, you may EITHER
- Use this sample to craft a notification message to send to your students and cc: uwturban@uw.edu
- OR ask Lisa to send that notification message to the class
- AND then be sure to let Abbey and Lisa know if you have made adjustments to class modality, and what notifications, if any have gone out to your students.
UW reporting protocols if a student informs you they tested positive for COVID-19
If a student reports to you they have tested positive for COVID-19 or believes they may have been exposed to the virus, you should report this information to Dr. Bernard Anderson, Associate Vice Chancellor for Student Life, at bander48@uw.edu and encourage the student to reach out to Dr. Anderson as well.
If this happens in a class which is meeting in person and you have been in physical proximity with the student, you should also contact UW’s COVID-19 Response & Prevention Team at covidehc@uw.edu or 206-616-3344.
In no circumstances should you share a student’s diagnosis or other private medical information with other students or colleagues.
