The Institute for Innovation and Global Engagement (IIGE) is home to the Global Honors Program and the Global Innovation and Design (GID) Lab. With campus serving academic programs as well as external facing revenue generating professional programs, the IIGE holds utmost its commitment to inclusive innovation and client confidentiality. Innovation and creative activities are respected and protected. In particular, the GID Lab provides campus and community workshops as well as executive education through a diverse team of expert facilitators and design practitioners that span the IIGE team, UWT faculty, staff, students, and external professionals.

As such, all personnel, whether in employment or voluntary capacity with the IIGE are mandated to fill out the form below to confirm agreement to the copyright and confidentiality terms specified. All questions are required, please read carefully.

Participate in a GID Lab orientation to understand terms and expectations of the partnership
Comply with all applicable rules, regulations, and policies of the University of Washington (UW) Policy Directory and Faculty Code.
Complete the Outside Professional Work for Compensation 1460 as applicable in accordance with Executive Order 57 (EO57) on Outside Professional Work Policy
Follow Executive Order 36 (EO36) on Patent, Invention, and Copyright Policy as applicable

Copyright Agreement
I acknowledge and agree that, in consideration for my employment in the IIGE and participation in GID Lab projects:

All copyrightable materials and inventions created or developed in the IIGE and GID Lab, and any materials or inventions created or developed using University of Washington resources, including but not limited to university facilities, equipment, personnel, or intellectual property, shall be governed by the University of Washington Policy on patent, invention, and copyright (Executive Order 36).
Any University-Owned or University-Sponsored Materials under Executive Order 36, as determined by the University in its sole discretion, may not be used, copied, distributed, displayed, or modified without the prior written approval of the University.
Any use of materials created or developed in the IIGE and GID Lab, whether in draft or final form, must have prior written permission from the IIGE and GID Lab, and must obtain and use IIGE and GID Lab authorship citation, respectively.
For GID Lab employees:
1. All materials, whether in draft or final form, I create or contribute in creating related to my work at GID Lab are produced for use by the GID Lab and that such materials and contributions made by me are works made for hire as defined by the United States Copyright Act of 1976 and United States copyright law.
2. If for any reason any such materials or contribution made by me are determined at any time not to be a work made for hire, I hereby assign to the University of Washington all right, title and interest therein, including all copyrights as well as renewals and extensions thereto.
3. If I wish to use any of the materials, whether in draft or final form, I create or contribute in creating related to my work at GID Lab, I must have prior written permission from the GID Lab and must obtain and use GID Lab authorship citation.
4. Upon the termination or expiration of my work with GID Lab, I shall return all materials I received, created, or contributed in creating, whether in draft or final form, to the GID Lab.
5. Under no circumstances may I use work created by myself or others in the GID Lab as my own, without written permission.
6. I have been advised by the University to seek independent legal counsel, and have had the opportunity to do so, prior executing this agreement.
7. Failure by me to comply with these terms may result in adverse action, including but not limited to, termination of my participation at the GID lab, and legal action for breach of contract and/or intellectual property infringement.

Confidentiality Agreement

I will hold all IIGE information including personnel information extending to all in employment with the IIGE in strict confidence and will exercise a reasonable degree of care to prevent disclosure to others.
I will hold all GID Lab client information in strict confidence and will exercise a reasonable degree of care to prevent disclosure to others.
I will not reproduce confidential information nor use this information commercially or for any purpose other than the authorized performance of my duties and/or my workshop participation in the GID Lab.
The GID Lab reserves the right to take disciplinary action, up to and including termination, for violations of this agreement in addition to pursuing civil or criminal penalties.

Personal Release Form

I hereby grant permission to the University of Washington (UW) and its assigns and licensees to take photographs or videos of me, and to make recordings of my voice. I give the UW permission to use these images, videos, and recordings, as well as my likeness, name, and voice, as follows:

The use may include reproduction, distribution, derivative works, display, and performance.
The use may be in composite or modified forms and in any media, now known or later developed, including without limitation newspapers, television, radio, the World Wide Web, and social media.
The use may be for any purpose throughout the world and in perpetuity, including, without limitation, education, trade, advertising, and promotion.

I further acknowledge that I will not be compensated for these uses, and that the UW exclusively owns all rights to the images, videos, and recordings, and to any derivative works created from them. I waive the right to inspect or approve of the uses of any printed or electronic copy. I hereby release the UW and its assigns and licensees from any claims that may arise from these uses, including without limitation claims of defamation or invasion of privacy, or of infringement of moral rights or rights of publicity or copyright.

This Release is binding on me, my heirs, assigns, and estate. The UW is not obligated to use any of the rights granted under this Release. This Release expresses the complete understanding of the parties.
