Request for Verification of Enrollment Status or Degree

This form may be used by students and alumni to request a verification of enrollment or degree, and will be submitted to the Office of the Registrar at reguwt@uw.edu. Allow a few days for processing. Note: please allow extra time for technical skills letters as they may take longer.

  • Third-party verifications: Submit your requests for enrollment verification to the National Student Clearinghouse (NSCH). If you are seeking verification for currently enrolled students, you will need wait until the third week of the quarter to make your request to the NSCH. Review the Academic Calendar to see when the quarters begin.

Instructions:

Step 1. Fill in your information on the form. (Student number, name, your major, you UW email address and a phone number where we can reach you.)

Step 2. (required) Select the type of verification you that you need: whether it's a form to be completed, or you need a letter of enrollment. 

  • Note: If you are sending forms, such as loan deferments, you will need to complete the borrower's section first before you submit your request.
  • For Technical Skills letters, please attach any forms you need completed and allow for extra time to complete this request. 

Step 3. (required for letter requests) Next, if you are requesting a letter of enrollment, indicate the type of letter needed. If you have submitted a request and have a form that needs completed, once you have uploaded your partially completed form, skip to #4. 

Step 4. (optional) Indicate any special instructions and/or any other items you need added to your letter.

Step 3. Type of Letter (select one):
Letter contains: registered status, full-time, part-time or not-attending, start & end of quarter, and number of registered credits
Letter contains: All of the information above in the Enrollment Status letter, plus, date accepted to major, name of major, school/program and CIP code for major
Letter contains: All of the information on the Enrollment Status letter, plus, Estimated tuition cost, class level, registered courses (includes: name of courses, course prefix and credits), and total credits
Letter contains: All of the information on the Enrollment Status letter, plus, Estimated tuition cost, class level, registered courses (includes: name of courses, course prefix and credits), and total credits
Letter contains: All of the information on the Enrollment Status letter, plus, Estimated tuition cost, class level, registered courses (includes: name of courses, course prefix and credits), and total credits
Letter contains: Degree received and graduation date
One Letter contains: All of the information on the Enrollment Status letter, degree received and graduation date. Second Letter: is completed within the School/Major on the specific skills and/or courses completed by the student.
