FOR STUDENTS: Schedule a Makeup Appointment Test
Makeup Testing Instructions
Please follow the makeup exam instructions below to schedule your makeup exam with OUE staff.
1. Confirm Instructor Submission
Please confirm with your instructor that they have submitted a Faculty Makeup Exam Request Form to OUE staff.

Exams cannot be scheduled until we receive the request from your instructor.

Same-day appointment requests are not accepted.
2. Determine Your Availability

Verify the testing window (first and last possible test dates) provided by your instructor.

Identify three (3) dates/times that work for you within that window.
3. Submit Your Request
Once you have your availability, schedule your exam by completing the form below:

Requests must be submitted two (2) business days before your preferred test date.

Late requests will not be scheduled, and you will need instructor approval to reschedule.

Please note: Students are responsible for scheduling their own exams.
5. Confirmation & Changes

Your appointment is confirmed when you receive a confirmation email from OUE staff. (your instructor will be copied).

OUE will review your request and schedule your exam based upon proctor availability.

If you are more than 15 minutes late, your appointment may be cancelled.

No-shows will be reported to your instructor, and additional approval will be required to reschedule.
6. Fees (if applicable)
There is no fee for current UW Tacoma students, unless:

UW student from another campus: $5

UW Online Learning student: $20
-
Non-UW student (other institutions/organizations): $40

Any required materials specified by your instructor
Note: We do not provide testing materials (e.g., scantrons, blue books, calculators, dictionaries).
Accommodations
If you require accommodation, please let us know at least 5 days in advance of the test date to receive testing accommodations. We will try our best to accommodate you or assist you any way we can. Faculty Instructions – Makeup Exams
The Office of Undergraduate Education (OUE) offers makeup exam proctoring as a courtesy to UW Tacoma faculty and students. Makeup exams are intended for students who are unable to take exams during scheduled class times and require an alternate, secure testing location.
Process to Submit a Makeup Exam Request

Submit a Makeup Exam Request Form at least five (5) business days before the requested exam date

Ensure exams are delivered to OUE at least two (2) business days before the student’s appointment

After submitting the form, please direct your student to schedule their exam by sharing the Student Appointment Request Form.

Students are responsible for scheduling their own appointment

Students must submit their request at least two (2) business days in advance

There is no fee for current UW Tacoma students
Testing Information
-
Monday–Friday, 9:00 AM – 4:30 PM
-
All exams must be completed by 4:30 PM
-
Appointments are based on proctor availability
-
Same-day appointments are not accepted
-
Requests submitted on Fridays for the following Monday cannot be accommodated

OUE is closed on weekends and university holidays

OUE does not proctor group makeup exams or full-class exams
-
OUE does not provide testing materials (e.g., dictionaries, calculators, blue books)
